REGISTRAION AND FEES
Attendance is strictly limited to 40 participants. The registration fee is $1000 per participant and includes admission to all sessions, course syllabus on CD, breakfast, lunch, am/pm breaks and dinner on Saturday evening.

PAYMENT OPTIONS: 
Accepted methods of payment are Visa, Mastercard, Discover or American Express. Registration confirmations will be provided online immediately following the registration process. If you would prefer not to register online, you may request a paper registration form by contacting Katherine Siler at 919-681-6437 or email siler006@mc.duke.edu.
CANCELLATION POLICY
A written notice of registration cancellation must be received by Friday, April 18, 2008. A cancellation fee of $100 will be assessed at that time. After this date, cancellation requests can not be honored and refunds will not be given.
