Registration and Fees
The collective course registration fees include breakfast each day, hot luncheon on July 6, 7, 8
and 9, and the conference reception on July 8th at 5 pm.

Rates (Per Person): 5 day conference curriculum including breakfast,
lunch, 4 workshops and evening reception. |

Payment Options: 
Accepted methods of payment are Visa, Mastercard, Discover or American Express. Registration
confirmations will be provided online immediately following the registration process. If you
would prefer not to register online, you may request a paper registration form by emailing your
request to confserv@notes.duke.edu.
Cancellation Policy
A written notice of registration cancellation must be received by Friday, June 20, 2008. A
cancellation fee of $75 will be charged. After June 20, 2008, no cancellation refunds will be
issued.
